- SUBMIT INTAKE FORM - click button below to fill out the online intake form.
- SCHEDULING - we will contact you for booking the assessment or training once we receive your completed form.
- SUBMIT PAYMENT - an invoice will be sent to you using the email address you provided on the intake form. Payment is required to secure your booking.

REQUIRED TRAINING SUPPLIES
THE FOLLOWING ITEMS WILL BE REQUIRED TO START OUR BRONZE PLAN TRAINING OR GROUP CLASSES.
- LEASH - 6 foot slip leash (non-retractable)
- COLLAR - Martingale type collar preferred
- RECALL LEASH - 15-20 foot
- TREAT POUCH - with treats
- CLEAN-UP BAGS - for "accidents"
*These items are NOT needed for the assessment.
Additional items will be required for Silver and Gold plan training.
